Starting up without a valid operating certificate
A permitted unit still cannot run until a valid operating certificate is in place. Commissioning schedules that assume startup on permit issuance often slip here.

Setting equipment without a complete application and seven-day notice
Placing a source on footings during NJDEP review is allowed only after a complete application is filed and a seven-day notice with its fee is submitted — and the unit cannot be tested or operated.

Making changes without the right revision or notice
Changes that require a permit revision, compliance plan change, seven-day-notice change, or amendment must go through that filing first; skipping it puts the permit out of compliance.

Assuming a permit moves with the equipment
Permits and certificates do not transfer to a new location or to a different piece of equipment, so relocations and replacements need their own filing.
